BASIC
NEEDS
ASSISTANCE
Thanks to support from our partners, this program provides limited financial support to LGBTQ+ Boulder County residents seeking assistance meeting basic needs including, but not limited to, groceries, gas, bill assistance, etc. Unfortunately, Rocky Mountain Equality is unable to offer hotel or housing vouchers at this time.
This confidential form is your way to share your needs with us so we can help you to access the assistance you deserve.
Completion of this form does not guarantee assistance. Assistance will be provided until funds run out. Funding is limited to Boulder County residents at this time. First come, first served. Please reserve funds for folks who have no or minimal access to resources.

Assistance Limits

One time assistance of up to $300 per person, paid directly to the vendor or provided on a gift card.

Verification Requirements

Address verification: One form required (utility bill, ID, shelter letter/email, paystub, bank statement, bill, rental contract, transcript or report card, motor vehicle registration, change of address form).

Income verification: Income received within the last 12 months (paystub, w-2, employer letter, federal income tax return document 2022/2023, SSI/SSDI letter, 1099).

CO-PAY
ASSISTANCE
Thanks to generous support from partners, our Sliding-Scale/Co-Pay Assistance Program provides short-term financial support to LGBTQ+ individuals in Boulder County for mental health care co-pays and/or sliding-scale fees. This program is open to people who are already working with a provider and to those who need help connecting with a provider.
This confidential form is your way to share your needs with us so we can help you access the assistance you deserve.

Assistance Limits

Up to three (3) months with a cap of $100 per session paid directly to the provider.

The provider must be willing to invoice and receive payments from Rocky Mountain Equality for your services.

Are you experiencing financial barriers to accessing your vital documents?
Rocky Mountain Equality’s Vital Documents Assistance Program aims to provide financial support to LGBTQ+ individuals seeking access to vital documents, such as state IDs, drivers licenses, and birth certificates. You must live in the state of Colorado to qualify for this program.
Completion of this form does not guarantee assistance. Assistance will be provided on a first-come, first-serve basis until funds run out. Please reserve these funds for individuals who have no/minimal access to financial resources.

Assistance Limits

The cost of documentation (including filing fees, where applicable).

Payments made directly to the vendor on your behalf (i.e., the Department of Motor Vehicles, VitalChek, etc.) via check or credit card.

Direct cash assistance will not be distributed to anyone for the purpose of obtaining vital documents.
